Q: What is a packet?

Just like a stapled packet of papers you might see in a paper-based office, FormDr can consolidate several forms into a group, called a packet. Each packet has a common theme or purpose. For example, a new patient intake packet could have: a demographics page, HIPAA form, health history, insurance form, consent forms, and other documents. The entire group of forms is sent to the patient through a secure, HIPAA-compliant process.  

Packets, just like forms, are customizable and streamline visits for your patients – and you. Each packet of forms has a unique link that is sent to the patient prior to the appointment. Instead of completing multiple separate forms with separate links, all can be completed at once, from the convenience of a phone, tablet, or computer.

We suggest using the convenience of packets to reduce the number of links/URLS you need to send to patients and to keep all the forms in one place. We believe in simple processes!          

Q: Can I put all of my forms into a packet, and then send each form out individually as needed?     

When you create a packet, it has one link/URL, so no – you can’t send an individual form directly from the packet. When you email, text, or place a packet on your website, the patient will receive all forms in the packet.

However – we understand that sometimes a patient doesn’t need the whole packet. In this case, you can simply place the needed form into its own packet to send. Special cases like these are easy to solve and we are always here to help.

Q: Why does it say unlimited forms, but then I am limited to packets?  

Think of a packet as a group of forms that is sent to a patient for a specific event – new patient registration, pre-op instructions, returning patient, etc. Within each packet you may have unlimited forms. To better understand this concept – think of how your paper forms are typically organized. Which ones are stapled together in groups?  Filed together?  Those are packets. You can put an unlimited number of forms inside each packet, simplifying your processes and giving your patients the convenience of completing packets from anywhere, anytime.
