Big News: FormDr is Getting a Major Upgrade – Starter Plan

Big changes are happening at FormDr – starting May 1, 2023, STARTER PLAN MONTHLY will be changing and also launching several new features to help your practice streamline your intake process, and business, like never before. New usage terms can be found here.

Plan changes: 

Effective May 1, 2023, STARTER PLAN usage will be changing and increasing in price (price increase for monthly customers only).

Monthly customers will increase from $29 to $39 a month. We want to offer you an opportunity to continue at the current price of $29 per month. By switching to an annual subscription, you can lock in the $29/month rate and enjoy the same great services for the next 12 months.

To update your subscription to an annual plan, simply go to Subscription under your Account Settings and change the billing cycle selection from MONTHLY to ANNUAL. Your new annual subscription will start at the next cycle and you’ll be billed $348 ($29/month) for the next 12 months.

But here’s some good news! We are also introducing several new features, all part of your existing plan. These additional features will greatly improve your patient experience, help streamline your practice and help you get even more value out of your subscription. 

Introducing 3 new features: 
  • Document Signing
    • Make any existing form digital, exactly as you want it, and have your patients sign/execute from any device, anywhere. 
  • Synced Forms
    • FormDr’s stand alone form option. Our solo Forms allow for a one off patient intake that can be sent, filled out and electronically returned. Forms can be imported and saved into multiple packets – making it a Synced Form. When updating a synced form, all packets using this form will be updated automatically. No more needing to make a change every time you use a saved Form.     
  • Custom Plans
    • Add on what your plan doesn’t have, select only what you need. This includes: Packets, Synced Forms, Document Templates and Assistant Users. For $5, add what you need to your existing plan to tailor it exactly to your practices’ needs.
