Acceptable Use Policy

Introduction

This Acceptable Use Policy (AUP) governs the use of FormDr’s Platform and Services. It applies to all users authorized under a Customer account. Updates to this AUP are periodically made at formdr.com/aup, and all users are responsible for staying informed of such changes. Terms not defined herein are as delineated in the Agreement. As used in this AUP, the term User applies to any user authorized by a Customer to access the Services.

Compliance Requirements

Do:

  1. Comply with Laws: Users must comply with all applicable laws, including those related to privacy (including HIPAA and HITECH), intellectual property, export control, tax, and regulatory requirements.
  2. Maintain Accurate Information: Users are required to provide accurate information and promptly update it as necessary.
  3. Review Policies: Users must regularly review and comply with FormDr’s Privacy Policy and any other policies related to the services.
  4. Compliance with Notices: Users must comply with any notices sent by FormDr concerning the services.
  5. Professional Use: Users must use the services in a professional, responsible, and lawful manner.
  6. Real Names: Users must use their real names and accurate information on their profiles.

Prohibited Activities

Users must not engage in activities that:

  1. Illegal Acts: Conduct or promote illegal activities.
  2. Harmful Conduct: Be harmful, fraudulent, deceptive, threatening, harassing, defamatory, obscene, or otherwise objectionable.
  3. Impersonation: Impersonate another person, user, or FormDr representative.
  4. Unauthorized Access: Attempt to access any other user’s account or misrepresent one’s identity or affiliation.
  5. Spamming: Send spam or other unsolicited communications.
  6. Disruption: Interfere with or disrupt FormDr’s services or servers.

Content Restrictions

Users must not post content that:

  1. Infringes Rights: Infringes any intellectual property or other proprietary rights of others.
  2. Violates Privacy: Violates the privacy or publicity rights of others.
  3. Harmful Content: Contains viruses, corrupted data, or other harmful, disruptive, or destructive files.
  4. Inappropriate Content: Is inappropriate, pornographic, or indecent.

System and Network Security

Users must not:

  1. Security Breaches: Violate the security of any computer network or crack any passwords or security encryption codes.
  2. Network Abuse: Make any network services unavailable to others through any means.
  3. Unauthorized Monitoring: Monitor any data or traffic on any network or system without authorization.

Intellectual Property and Brand Protection

Users must:

  1. Respect FormDr’s Marks: Not use FormDr’s trademarks, service marks, logos, or branding without prior written consent.
  2. Avoid Infringement: Not engage in activities that infringe on the intellectual property rights of FormDr or any third party.

Enforcement and Violations
FormDr reserves the right to investigate any violation of this AUP and take appropriate remedial action, including but not limited to suspending or terminating Customer’s (or any Customer user’s) access to FormDr’s services. Customer (or its users) found to violate this AUP may also be subject to legal action.

Updates to the AUP
FormDr may update this AUP from time to time. If you do not raise any objections within fifteen (15) days of notice of changes, continued use of the Services after such changes shall constitute acceptance of the new AUP.
