Intake forms with required fields. Yes or no?

Let us start with a quick YES! Moving forward with a paperless intake system is a huge step in the right direction, but going online isn’t enough. We often take the first few steps towards improvement only to find ourselves asking more questions like: “What’s next?, Did I do this right? Will this work for my business?” With the growing number of online intake forms out there, you will often find yourself with an obscene amount of information or even worse, no information at all.

Adding the required field to your intake form will help reduce the stress of incomplete forms. The key is to start identifying the problem areas in your intake process. Open your intake forms online and review them to make sure you are asking vital questions. You would be surprised at how often an intake form includes questions like: “What is your date of birth?” and “How old are you?” Yes, they are two different questions but both get the same end result. If you are having to ask the same patient/client the same question twice, then you aren’t asking right.

Although these questions may seem harmless, they are in fact going to affect your return rate. Eliminate useless questions and start making important questions required. It’s extremely easy and gets you the information you need. Just be careful not to go overboard with your required questions. Filling an entire form with required questions is enough to make clients scream and head for the hill. After all, any question can be asked at a later time or even with another form.

Here are a few questions that are always good to make required:

  • Name
  • Address
  • Gender
  • Date of birth
  • Phone
  • Email
  • And any question you feel is vital to their visit
