A Fair Refund Policy

At FormDr, we believe in fair, straightforward policies. If you’re ever unhappy with your subscription, contact our support team — we’re here to help.

Refunds & Credits

We review refund requests on a case-by-case basis, prioritizing fairness and transparency. We may offer credits or prorated refunds in the following situations:

  • You were just charged for a renewal but intended to cancel — we’ll refund the charge.
  • You haven’t used your account in recent months — we’ll review and may offer a refund.
  • You subscribed to an annual plan but didn’t get value from it — we’ll refund the unused portion.
  • Your account was charged after requesting cancellation. — we’ll refund the charge.
  • You were billed despite unsuccessful payment method updates. — we’ll refund the charge.
  • Extended downtime or delayed support responses. — we’ll review and issue a credit.
  • Billing errors or duplicate charges. — we’ll refund the charge.
  • Miscommunication or delays during onboarding. — we’ll review and issue a credit.
  • Features not working as expected due to platform bugs. — we’ll review and issue a credit.
  • Temporary disruption of integrations (e.g., EHR/EMR). — we’ll review and issue a credit.
  • Delays in custom form setup promised within a timeframe. — we’ll review and issue a credit.

Plan Changes

Customers on Platform, Teams, or Legacy plans cannot change to a different plan. If you no longer wish to continue with your current plan, you must:

  1. Cancel your account — all associated data, including forms, documents, and patient records, will be permanently deleted.

  2. Start a new account — previous data and configurations will not be carried over.

  3. Rebuild any forms manually — any forms built by the FormDr team will not be transferred or recreated.
