Core Plan
Your Core Plan includes:
- Unlimited Forms & Packets – Build as many forms and packets as your practice needs. No caps on how many intake, consent, or history forms you keep active, so you can create a dedicated packet for every visit type, provider, or specialty instead of reusing one generic form.
- Unlimited Documents & Signatures – Build a template for every form your practice uses and collect as many signed documents as you send. There are no template caps and no per-signature fees; your plan scales with your active patient count, not with how much you use it.
- Unlimited Assistant Users – Add your whole team. Front desk, billing, and clinical staff each get their own login to send forms and view submissions, with no per-seat cost and no sharing credentials.
- Form Conversion Service – Send us your existing paper or PDF forms and our team rebuilds them as digital form packets. No setup work on your end — you keep the forms your practice already uses. Up to 25 pages converted for free.
- Enhanced Support – Priority access to our support team, with faster response times and hands-on help with form building, workflow setup, and troubleshooting. Get up to 2 scheduled training meetings with a live FormDoctor representative.
- Automated Appointment Reminders – Patients automatically receive reminders before their appointment by text and email, reducing no-shows without anyone on your team sending them manually.
- Text & Email Invites – Send form requests by SMS as well as email. Patients get a link on their phone and complete paperwork before they arrive, which is where most of the completion-rate lift comes from.
Your Core plan includes 50 active patients per month. A patient becomes active the first time you interact with them in a billing month — sending a form or packet, texting or emailing them, or collecting a signed document. After that, everything else you do with that patient that month is included: additional forms, follow-up messages, a second signature request, a rescheduled appointment. One patient counts once, no matter how much paperwork or messaging they generate. Patients you don’t touch in a given month don’t count toward the limit, the count resets each billing cycle.
